You can invoice Medicare for reviewing and signing the document. To bill for Plan of Care Oversight, the attending physician will need to do the following:

 

• Review the HCFA Form 485 sent by the agency.

 

• Sign in box #27 (first page), and #9 (second page).  Per Medicare guidelines, the attending physician's signature is the ONLY accepted signature.

 

• Your billing department can request 485 copy by calling 626-732-6542 and ask for Medical Records.
