OUR SERVICES

SKILLED NURSING

The Skilled Nurse is a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) with training and experience in providing care in the home. The nurse communicates frequently with your physician to update your plan of care. Skilled nurning may include treatment, injections, wound care, IV fluids, antibiotic therapy and family and patient health teaching and counseling.  Performs comprehensive body assessment and recognize reportable conditions to doctors.

CERTIFIED HOME HEALTH AIDE (CHHA)

Home Health Aide services are delivered under the supervision of a registered nurse or a licensed therapist. An Aide is assigned when there is a specific need for personal care on a part-time basis at home.  An Aide assists patient with meal preparation, personal care and hygiene, light housekeeping, ambulation and exercises under professional supervision.  Any additional duties the home health aide performs will be planned by you and the nurse and added to your plan of care.

PHYSICAL, OCCUPATIONAL and SPEECH THERAPISTS

Physical, Occupational and Speech Therapy services are provided by a licensed therapist or licensed therapy assistant under the direction of the therapist. Your therapist will provide specific information about the services and treatments you will receive.

 

MEDICAL SOCIAL WORKER

The Medical Social Worker provides short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, finances and long-range planning.

SUPPLIES / THERAPY / EQUIPMENT

Supplies/Therapy/Equipment: Medical Supplies, Therapy Services or Durable Medical Equipment (DME) may be required to carry out your plan of care. All medically necessary therapy services or medical supplies must be coordinated with the home health agency while you are receiving Medicare covered home health services. If you arrange for these services or supplies on your own while under our plan of care, Medicare will not reimburse you or other suppliers.

Our staff is able to assess the make arrangement for obtaining medically necessary equipment as prescribed by the physician, such as hospital beds, wheelchairs, commodes, shower chairs, canes, tube feeding equipment, oxygen, suction machines, etc.  that are available under the patient’s insurance plan, and may arrange for the delivery of such DME to patient’s house.

NUTRITIONAL GUIDANCE

Assess nutrition status and assists with individualized meal planning.  Teach and instruct patient, family and caregivers techniques on how to implement dietary plan of care.

PHARMACY

Our staff is able to coordinate the pharmaceutical needs of the patient such as I.V. Therapy, injectable medications, diabetic supplies, etc.

LABORATORY WORK

Our staff is able to coordinate laboratory work ordered by the patient’s physician.

OTHER

Our company can provide interpreter service for the patient with impairments in communication, e.g., deaf, hearing or speech impaired, or Limited English Proficiency (LEP) patients through the Language Line Services
